Uploading Required Documentation
Before you submit a contact for a background check through your Aspen iRecord® account, required documents will need to be completed for this individual within their contact profile.
Step 1: Login to your Aspen iRecord® account and click on the Contacts tab.
Step 2: Click on the contacts name that you will be submitting for a background check, their name is a hyper link and it will take you to their contact profile. All required fields need to be updated. Find the section called "Background Check Documents", click on the provided download links Download written consent form template and Download SSA-89 Form. (Some information will be pre-populated on the forms)
Step 3: Print the forms that you downloaded, fill them out and wet sign them. No electronic signatures.
Reminder: You must ensure that both pages of the completed and wet signed SSA-89 form are uploaded. For your security, once uploaded, this uploaded form can’t be downloaded again. You should keep a copy of the completed SSA-89 form for your own records in the event you need to review at any point in the future.
Step 4: Upload the completed forms to the appropriate upload link just below the download links. The written consent form needs to be uploaded to the Upload Written Consent Form link and the SSA-89 form needs to be uploaded to the Upload SSA-89 Form link.
Step 5: Click update in the bottom right corner of the screen and you will be navigated back to the Contacts tab.
